We advise all patients to verify coverage with their insurance provider as any and all financial liability ultimately is the patient's responsibility. Physician Referrals
Delta Sleep Medicine of Long Island provides the highest quality sleep care to your patients. To process a referral, please complete our Physician Sleep Evaluation Request Form and either email it or fax it to us at (631) 343-7245. If you have a copy of your patient's insurance card or clinic notes, please send those as well. We will then set up the appointment.
Alternatively, you may provide the patient with our contact information and ask the patient to contact us directly, mentioning your office. We will report back to you once an initial evaluation with Dr. Lown has been performed.
